Job summary

Premier Home Health Providers Inc. seeks a compassionate Home Health Aide to deliver in-home personal care under the supervision of a Registered Nurse. You will assist with daily activities, monitor health, and document care during per diem visits across patients’ homes.

Ideal candidates have a CA CHHA certificate, CPR certification, and reliable transportation. The role offers flexible scheduling and opportunities for advancement within a family-oriented team.

Qualifications

• Californian Certified Home Health Aide certificate is required.
• Current CPR certification with AHA is required.
• Valid driver’s license with reliable vehicle is required.
• At least 18 years old and able to read and follow written instructions.
• Ability to work independently with minimal supervision and maintain hygiene standards.

Responsibilities

• Provide personal care including bathing, grooming, oral hygiene, dressing, and skin care.
• Assist with mobility, exercises, and range-of-motion activities per care plan.
• Support patients with meals from preparation to feeding assistance.
• Take and record vital signs as directed.
• Assist patients with medication self-administration.
• Keep patient living space clean, safe, and orderly.
• Observe patient condition and report changes to supervising nurse.
• Document care provided according to organizational standards.
• Perform other duties as needed for patient services.
