Job summary

Qualifications

• Doctor of Pharmacy or BS Pharmacy degree required.
• Hospital experience preferred.
• Pharmacy state licensure within 60 days of start date; BLS or ACLS required; PALS for pediatric practice.

Responsibilities

• Prepares and dispenses drug orders per physician request according to established policies and protocols.
• Interprets verbal and written drug orders and transcribes into the patient medication record; maintains accurate records.
• Compounds and dispenses pharmaceuticals including non-sterile and sterile products, chemotherapy and parenteral nutrition.
• Issues controlled substances and maintains records as required by law and policy.
• Ensures safe, appropriate, and cost-effective drug therapies; monitors for interactions and dosing adequacy.
• Supervises pharmacy support personnel and verifies technicians' daily activities.
• Ambulatory care pharmacist providing care via approved protocols and collaborative agreements, including anticoagulation management, patient education, and discharge reconciliation.
• Participates in quality improvement activities and clinical interventions as defined by department goals.
