Yale New Haven Health is seeking a Dietitian to develop and implement nutrition care plans based on patient assessments and to counsel patients and families on dietary needs and lifestyle adaptations. The role involves documenting nutrition data, participating in rounds, and ensuring diet orders and menus align with care plans.

The position requires a B.S. in Dietetics or related science, with ADA registered dietitian licensure preferred and at least one year of experience.

Responsabilidades

• Develops And Implements Plans Of Care Based On Assessment Of Nutritional Needs Of Patients And Correlates Such Plans With Other Health Care Activities.
• Counsels Patients And Families On Their Diets, To Include Nutritional Principles, Food Selections, Economics And Adaptation To The Patients Lifestyle.
• Communicates Appropriate Dietary History And Nutrition Care Data Through Written Records Systems. Documents Information Concerning Nutritional Care In Patient Medical Records.
• Participates In Multidisciplinary Patient Care Rounds/Team Meetings As Assigned.
• Evaluates Food Served For Conformance To Quality Standards And Diet Prescription.
• Consults And Counsels The Patient Nutrition Representatives Regarding The Following For Which They Are Responsible, Processing Of Diet Orders, Menus And Other Directives Related To Patient Care Modification Of Menus According To Established Patterns And Procedures Of All Aspects Of Floor Supervision.
• Meets Performance Expectations For Customer Service, Teamwork, Resource Utilization, And Staff And Self Development As Outlined In Performance Review.
• Performs Other Duties As Assigned Or Directed To Ensure Smooth Operation Of The Department/Unit.
