New Alternatives for Children, Inc. (NAC) is seeking a Per-Diem Service Provider to support CFTSS and HCBS services across New York City. The role involves delivering a range of services in homes, schools, and community settings.

You will work under supervision and may provide Caregiver/Family Support, Respite, and psychosocial rehabilitation among other duties. Candidates should have a high school diploma, with a bachelor’s preferred for some services, and may require a master’s for

Qualifications

• Must be willing to work in participants' homes.
• Must have quick and easy access to a computer and be adept at using internet, word processing and email.
• Bi-lingual (Spanish speaking) staff preferred.

Responsibilities

• Demonstrate skills that promote a safe environment for children and families in living environments and the community.
• Provide services including Caregiver/Family Support, Respite, Prevocational and Supportive Employment, and more.
• Develop Service/Treatment Plans with goals and report progress.
• Assist in crisis response and management as necessary.
• Work flexible hours including weekends and evenings as required.
• Complete Medicaid billing documentation within prescribed timeframes.
• Complete required trainings and respond to communications within 24 hours.
• Participate in supervision as scheduled.
