Job summary

Affinity Home Health Care Inc is seeking a Physical Therapist to assess patients and provide evidence-based physical therapy. You will evaluate patients through clinical assessments, create tailored care plans, deliver treatments grounded in best practices, and document progress and outcomes.

Responsibilities include coordinating with physicians and clinical staff, and supervising assistants as needed. Pelvic floor therapy experience is preferred but not required.
