Job summary

Foundations Health seeks a Certified Peer Support Specialist for an OTP Clinic grant-funded role. The specialist will support individuals reentering the community after incarceration, leveraging lived recovery experiences to promote resilience and reduce recidivism.

Responsibilities include coordinating with clinicians, case managers, and reentry staff to provide outreach, intake, and wrap-around services for MAT program participants, while maintaining confidentiality and reporting requirements.

Qualifications

• High school diploma or GED required.
• Associates degree preferred.

Responsibilities

• Provide one-on-one peer support to reentry and jail diversion clients, sharing recovery experiences to inspire hope.
• Facilitate peer support groups and educational workshops focused on recovery, coping skills, and community resources.
• Collaborate with multidisciplinary teams to coordinate care.
• Help develop individualized reentry plans supporting clients’ goals and recovery journeys.
• Maintain documentation of support activities and client progress per program requirements.
