Job summary

The South Shore Association dba SAIL in Baldwin, NY seeks a Recovery Coach to help clients move toward stability, using personal recovery experience to model progress.

You will connect clients with community resources, support housing partners, provide counseling on medication and coping, and assist with WRAP planning and follow-up after discharge. This is an on-site role at 1976 Grand Ave, Baldwin, NY.

Qualifications

• High School Diploma preferred.
• 3+ years experience as a mental health recipient and role model.
• Certification by the Academy of Peer Services preferred or in progress.

Responsibilities

• Support clients' community integration and socialization with resources and activities.
• Assist housing partners in accessing outside supports, including Wrap-Around services.
• Provide counseling and education on medication, symptoms, coping, and recovery.
• Develop Wellness Recovery Action Plans (WRAP) and follow up after discharge.
• Coordinate services with treatment teams and housing staff; maintain communication.
• Participate in supervision, staff development, and trainings when scheduled.
• Respond to emergencies, complete reports on time, and perform other duties.

Qualification and Competencies

• High School Diploma preferred.
• At least 3 years experience as a mental health recipient, who has successfully utilized opportunities for personal growth. Ability to respond appropriately to supervision and work as a team member.
• Certified by the Academy of Peer Services or undergoing certification is preferred.

General Description

Will act as a recovery coach to assist clients in working toward stability. Will utilize past experience and success as a recipient in order to serve as a role model to Supported Housing clients.

Duties and Responsibilities

• Support clients' community integration and socialization by connecting them with appropriate community resources and activities.
• Assist clients and partner housing agencies in accessing outside supports, including Wrap-Around services through the Nassau County Office of Mental Health.
• Provide supportive counseling and education on medication and symptom management, coping strategies, and recovery.
• Work with clients to develop Wellness Recovery Action Plans (WRAP) and provide follow-up after discharge.
• Coordinate services by maintaining communication with treatment teams, Health Home care coordinators, and housing staff.
• Participate in supervision, staff development, and work-related workshops or conferences as scheduling permits.
• Respond appropriately to emergencies, complete required documentation and reports on time, and perform other assigned duties.
