Job summary

RISE Life Services is seeking a part-time Peer Support Specialist for Elevations Clinic in Riverhead, NY. This role focuses on person-centered mental health support, mentorship, and wellness coaching within a licensed clinic.

You will document services in the EMR, comply with HIPAA, and assist clients in reaching goals, gaining life skills, and connecting to community resources. A high school diploma and lived experience are required; bilingual Spanish a plus.

Qualifications

• High School Diploma or equivalent (required).
• Lived experience relevant to peer support.
• Provisional Peer Specialist Certification and credential hours within requirements.
• Prior experience in behavioral health or peer support settings preferred.
• Familiarity with EHR systems such as eClinicalWorks preferred.

Responsibilities

• Provide billable 1:1 peer support services during daily scheduled hours on-site and/or in the field.
• Support clients in developing personal goals, coping strategies, life skills, navigating services, improving self-management, and reconnecting with care.
• Use lived experience to foster hope, validation, and mutual understanding in a non-clinical capacity.
• Assist clients with connecting to appropriate community resources and supports.
• In collaboration with the clinical team, support client safety planning and other wellness-focused strategies within the scope of the peer role.
• Based on client need and interest, facilitate or co-facilitate peer support groups (Wellness check-ins, Wellness planning, Coping skills, Psychoeducation, Stress management).

GENERAL STATEMENT OF DUTIES:

The Elevations Clinic Treatment Program Peer Support Specialist provides person-centered, mental health and wellness focused support within a licensed behavioral health clinic. The Peer Support Specialist uses lived experience to offer supportive services that promote hope, self-advocacy, and connection. The Peer provides mentorship, emotional support, wellness coaching, and assistance with developing coping strategies, goals, and life skills. Based on client interest, the Peer may also facilitate or co-facilitate peer support groups.

KEY ESSENTIAL FUNCTIONS:

Peer Support Services (Individual & Group)

• Provide billable 1:1 peer support services during daily scheduled hours both on site and/or in the field.
• Support clients in developing personal goals, coping strategies, life skills, navigating services, improving self-management, and reconnecting with care.
• Use lived experience to foster hope, validation, and mutual understanding in a non-clinical capacity.
• Assist clients with connecting to appropriate community resources and supports.
• In collaboration with the clinical team, support client safety planning and other wellness-focused strategies within the scope of the peer role.

Outreach & Reengagement

• Conduct outreach to clients who have missed appointments or disengaged from services, using phone calls, EMR messaging, or other approved methods.
• Coordinate with clinical providers to reengage clients and assist in scheduling or service connection.
• Participate in approved community outreach activities and assist with connecting individuals to available clinic and community resources.

Documentation & Compliance

• Accurately document all peer support services and outreach activities in the EMR (e.g., eClinicalWorks), following agency and payer requirements.
• Maintain strict compliance with HIPAA, confidentiality standards, and ethical peer support practices.
• Accurately manage and update records in both electronic and physical formats.
• Support consistent and organized clinic operations through reliable documentation and communication.

QUALIFICATIONS:

Education & Experience

• High School Diploma or equivalent (required)
• Lived experience
• Provisional Peer Specialist Certification and hours towards professional certification within the requirements of the credentials
• Prior experience in behavioral health or peer support settings preferred
• Familiarity with Electronic Health Record (EHR) systems (e.g., eClinicalWorks) preferred
