Job summary

Jobtailor seeks a Peer Recovery Specialist to provide recovery-oriented support to patients with substance use disorders and manage a caseload in residential and outpatient programs.

You will conduct assessments, collaborate to develop individualized recovery plans, and document in the electronic health record. Travel to meet patients and attend trainings may be required.

Qualifications

  • High school diploma or equivalent required.
  • At least 1 year of mental health and/or substance use knowledge/experience.
  • Must complete supervised work experience and obtain Peer Recovery Specialist certification within 1 year of hire.
  • Ability to engage patients using recovery experience.
  • Strong interpersonal, communication, and listening skills to assess needs and develop plans.
  • Ability to work independently and in a multidisciplinary team.
  • Familiarity with recovery principles, trauma-informed care, and harm reduction.
  • Proficient use of electronic health records for treatment documentation.
  • Effective written and verbal communication; respond to inquiries promptly.
  • Willingness to travel to various locations by vehicle for meetings/trainings.

Responsibilities

  • Provide recovery-oriented support services to patients with substance use disorders
  • Maintain a caseload for patients in residential and outpatient treatment
  • Conduct assessments to identify support service needs
  • Collaborate with patients to develop individualized recovery support plans emphasizing purpose, mindfulness, and personal goals
  • Provide 20–25 hours weekly of direct in-person, telephonic, or virtual peer support
  • Maintain accurate recovery-plan, intervention, resource, and progress-note documentation in the EHR
  • Identify community resources and connect patients with appropriate services
  • Assist with coordinating transportation to supported community services
  • Identify relapse-prevention strategies and help patients manage triggers and high-risk situations
  • Help patients build and sustain recovery-oriented social networks
  • Participate in interdisciplinary case staffing meetings to discuss care coordination
  • Contribute to peer recovery team meetings evaluating program effectiveness and patient progress

Requirements

  • Demonstrated ability to leverage recovery experience to engage patients during their recovery journey
  • Demonstrated interpersonal, communication, and active listening skills to assess individual needs and develop individual support plans
  • Ability to work independently and as part of a multidisciplinary team
  • Familiarity with recovery principles, trauma-informed care, and harm reduction approaches
  • Ability to learn and become proficient in using electronic health records to enter and maintain treatment documentation
  • Ability to communicate efficiently and effectively with others, in groups or individually, to gather and present information
  • Ability to respond to telephone and email inquiries in a timely and professional manner
  • Ability to use a keyboard and display screen to receive, retrieve, and/or review information and data on a regular basis
  • Ability to continually proofread, check and verify data and information from printed to digital documents
  • Ability to travel to various locations by vehicle to meet with patients and/or attend meetings and training

Proficient in maintaining accurate documentation in electronic health records and connecting patients with community resources.
