Transformmn is looking for a Transformation Coordinator in Minneapolis to support residents on their journey towards community integration. This full-time role involves maintaining client-focused services, creating individual service plans, and facilitating transportation for residents.

Qualifications

• Lived experience with addiction and recovery.
• No less than two years of documented freedom from harmful chemical use and probation.
• Experience collaborating with people in re-entry and/or recovery.

Responsibilities

• Execute and maintain client focused reentry/transformational services.
• Collaborate with residents to create and update Individual Service Plans.
• Facilitate transport for residents around the Rochester area.
