Job summary

Chapters Health System seeks a Chaplain to deliver spiritual care to patients and families enrolled in its programs, and to support staff and volunteers. The role requires theological education and ordination, with EMR experience and a willingness to travel in patient care settings.

The Chaplain will develop care plans, coordinate with the care team, and act as liaison with local clergy to fulfill the organization’s mission and standards of practice.

Qualifications

• Master's degree in Divinity, Theology, Christian counseling or related degree from an accredited institution.
• If hired before 5/26/2024, Bachelor's degree in Divinity or related degree from an accredited institution.
• Ordination or commissioning/endorsement from a recognized ordaining body.
• One unit of Clinical Pastoral Education (CPE) or in process within six months of employment.
• Experience with EMR/EHR systems preferred; appropriate computer skills required.
• Must have a valid driver's license and reliable transportation; ability to travel.
• Must be able to communicate clearly in English.

Responsibilities

• Develops a spiritual care plan in conjunction with the interdisciplinary group based on patient/family traditions and assessments.
• Provides spiritual care and initiates interventions to resolve care plan issues.
• Implements the spiritual care plan through personal contact or volunteers/clergy as appropriate.
• Monitors and reassesses the care plan; maintains documentation with timeliness and accuracy.
• Liaises between the Company and local clergy; participates in interdisciplinary team meetings.
• Performs other duties as assigned.
