Job summary

Qualifications

• Master's in Social Work required.
• Current Florida social work licensure.
• LCSW required.
• Three to five years of recent clinical experience (population-specific preferred).

Responsibilities

• Patient and Family Services: Identify patients and families needing coordination of community support services by screening medical records, attending team meetings and referrals from clinical staff.
• Medical Case Management Services: Plan and manage case management to enhance continuity of care and wellness, coordinating referrals to external agencies as needed.
• Crisis Management: Respond to emergencies with appropriate interventions to stabilize situations during working hours.
• Documentation: Document services rendered in the medical record and maintain clinic information system records.
• Professionalism: Adhere to NASW code of Ethics and hospital policies to ensure quality patient care.
• Administrative Duties: Attend meetings, participate in committees, and complete required reports.
• Age-Specific Competency: Deliver care appropriate to patient age and developmental status.
• Psychotherapy Services: Provide assessment, diagnosis and treatment for mental health concerns on a referral basis.
• Contributes to clinic operations by performing related duties as required.
