Cincinnati Children's Perlman Center is seeking a Social Worker to join the interdisciplinary team, conducting psychosocial assessments, providing care coordination and family support for CP patients. The role emphasizes collaboration with therapists, physicians, and educators in a pediatric rehab setting.

The position is based at Burnet Campus with occasional satellite visits, with a 40-hour work week from 8 AM to 4:30 PM, Monday through Friday, and regular status in a dynamic care environment.

Qualifications

• Master’s degree in Social Work
• LSW License in the State of Ohio
• Previous experience is highly desired

Responsabilités

• Psychosocial Assessment- Performs psychosocial assessments taking into consideration family, social, and medical history.
• Resource Support- Collaboration with the health care team in the development of a treatment plan and assisting in the identification of appropriate resources to support the interventions.
• Psychosocial Interventions- Provide psychosocial services as identified in the patient's comprehensive plan of care.
• Interdisciplinary Collaboration- Working for system improvement, promoting patient/family/caregiver well-being.
• Collaboration- Partner with the patient, family, healthcare team in the safe transition of care to the next, most appropriate level.
