Job summary

Mass General Brigham Community Physicians, Inc. is seeking a licensed clinical social worker for a full-time, hybrid role in Massachusetts. You will conduct psychosocial assessments, collaborate with the interdisciplinary team, and contribute to individualized treatment planning for patients and families.

Required qualifications include an MSW or related master's degree, 0-1+ years in a medical setting, and licensure (LCSW/LICSW/LMHC). Pediatric experience is highly preferred.

• 0-1+ years of clinical experience in a medical setting required.
• Class D Driver's License required.
• Licensed Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) required.
• Pediatric experience strongly preferred.

Responsibilities

• Provide psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues; formulate biopsychosocial assessment, disposition, and treatment plans.
• Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
• Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.

Schedule and Work Model Required

• Full Time (40 hours) Monday through Friday
• Hybrid: PAGS (Pediatric Associates of Greater Salem) at 30 Tozer Road, Beverly MA (~2 days per week); 84 Highland Avenue, Salem, MA (~1 day per week); 376 Boston Street, Topsfield, MA (~1 day per week); remote from home (~1 day per week)
• Remote work requires stable, secure, quiet, compliant work station
• Team meetings on-site at Assembly Row in Somerville, MA on the first and third Wednesday of every month
