Job summary

Homeside Rehab is seeking a compassionate Speech-Language Pathologist (SLP) to join our pediatric home health team in Kew Gardens, NY. You will provide individualized, evidence-based speech therapy services to patients in their homes with flexible scheduling.

The role offers part-time, per diem hours, opportunities to collaborate with a supportive clinical team, and a focus on home-based care for children with speech, language, fluency, articulation, and voice disorders.

Qualifications

• Active NYS Speech-Language Pathologist license.
• Minimum 1 year of experience in Home Health, Outpatient, or SNF settings preferred.
• Basic computer literacy for documentation and communication.
• Strong clinical and interpersonal communication skills.
• Ability to work independently in a home health environment.
• Reliable transportation for home visits preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on patients’ needs and goals.
• Provide evidence-based treatment for speech, language, fluency, articulation, and voice disorders.
• Maintain accurate and timely documentation of patient care.
• Collaborate with the clinical team to ensure coordinated and effective treatment.
• Communicate effectively with office staff, patients, and caregivers.
• Collaborate with healthcare professionals to ensure comprehensive care.
