Job summary

KidsCare Home Health is hiring a Skilled Nursing Registered Nurse responsible for the delivery and supervision of patient care within the skilled nursing line, covering assessment, planning, implementation, evaluation and coordination of care. You will own and coordinate care for SN caseloads, perform Start of Care and Recertification visits, and develop plans of care for patients.

The role requires a valid state RN license, a valid driver's license, CPR certification, and preferred BLS.

Responsibilities

TheSkilled Nursing Registered Nurseis responsible forthe delivery and supervision of patient care targeting the Skilled Nursing Line of Business through the nursing processes of assessment, diagnosis, planning, implementation,evaluationand coordination of care.

Essential Duties and Responsibilities

• Ownand coordinatecare fortheskillednursing casesto include allassessments, recerts, anddischargesvisitsto patients requiring intermittent professional nursing services.
• PerformStart of Care and Recertificationvisitsfor patients.
• Perform hospital assessmentandestablishplansofcareonnew and existing patients.
• Monitor and supervise the nursing care of patients within assigned territory.
• Collaborateand communicatewith the casemanager anddirect patient care staffto ensure quality patient care deliverynecessaryto achieve and exceed individual goals.
• Provide patient and family education related to diagnosis, treatment, medications, and prevention; collaborate with interdisciplinary team members to ensure coordinated, quality care.
• Deliver services in compliance with physician orders, state and federal regulations, accreditation standards, and agency policies.
• Attendall IDT and mandatory meetings, as well as staff meetings, team conferences, and educational in- services.
• Participate in an on-call rotation as a member of the case management team.
• Adhere to productivity guidelines for skilled nursing.
• Always adhere to infection control and safety protocols.
• Provide care in environments where airborne precautions may berequiredandmust be able to wear a fit-tested N95 respirator or higher-level respiratory protectionin accordance withOSHA 29 CFR 1910.134.
• Successfully complete andmaintainOSHA-required medical evaluation and respirator fit testing. The ability to wear required respiratory protective equipment and perform all patient care duties while wearing such equipment for extended periods is an essential function of this position.
• Maintain professional licensure and required certifications.
• Perform other job-related duties as assigned consistent with business needs.

Qualifications

Education and Work Experience Requirements

• Validstatedriverlicense and reliable transportationwithabilityto travel to patient homes.
• CPRcertification isrequired. BLS Healthcare certification preferred. (must bemaintainedthroughout employment).
• Strong clinical assessment and critical thinking skills.
• Ability to work independently in home-based settings.
• Effective verbal and written communication skills.
• Ability to respond appropriately in emergency situations.

Other Qualifications and Skills

• Current active Registered Nurse license in the state of practice (or compact license whereapplicable).
• Two(2) yearsofRegistered Nurse experience in a clinical care settingpreferred.
• One (1) year ofpediatricexperience preferred.
• One (1) year of home health experienceis preferred.

• Ability to tolerate wearing respiratory protective equipment for prolonged periods.

• Pushing and pulling up to 50 pounds (e.g.,positiongpatients moving equipment).
• Supporting up to 50 pounds (e.g., ambulating patients).

• Carrying equipment and medical supplies.
• Visual acuityis sufficientto read monitors, medical devices, and documentation (within 24 inchesor less).

Work is performed primarily in private home settings where environmental conditions mayveryand are not under agency control.

Driving Responsibilities

This position requires driving on behalf of the company. Qualified applicants and team members driving personal vehicles while performing job duties for KidsCare will:

• Be at least 18 years of age.

• Possess andmaintains a valid state driver's license.

• Have at least one (1) year of US driving experience.

• Not have engaged in (or have been cited for) any of the following or similar behaviors within the last five (5) years:

• Driving while impaired or driving while in possession of a controlled substance for which the driver does not have a current valid license or prescription.

• Failure tocompliance with implied consent law (refusing a test for impairment),

• Committing a Misdemeanor or Felony involving a motor vehicle. KidsCare Home Health adheres to all federal and state mandatesregardingthe review of MVR checks and has an established policy for reviewing andadjudicatingany convictions or charges found. As a result, KidsCare reserves the right to suspend or separate employment strictly based upon the information provided and its potential impact upon organizational liability related to performing specific job required responsibilities.

• Showing a pattern of dangerous driving habits in MVR Records

• Failing to followlaw enforcement instruction

• Not having more than three (3) minor moving violations. Examples include speeding, failure to yield the right of way, unsafe lane changes, child safety seat violations, etc.

• Agree to regular driving history searches as requested by the company (refusal tosubmitto driving history searches may be grounds for separation).

• In addition to the above, the company may, in its solediscretiondetermineother behaviors as “at-risk” which may lead to revocation or restriction of company driving privileges. (NOTE: Individuals receiving company owned (fleet vehicles) may haveadditionalrequirements. MVRs will be reviewed during the fleet process.

Pay Range for this role is $60-75 per session and is based on skill set and experience.
