Job summary

Phoenix Home Care and Hospice seeks compassionate LPNs and RNs to deliver in-home pediatric and young adult nursing care. You will build strong relationships with patients and families, adapting care to each visit and milestone with a focus on comfort and safety.

Qualifications

• Current LPN or RN license is required.
• Pediatric experience preferred, but training is provided.
• Demonstrates passion for patient-centered care.

Responsibilities

• Provide one-on-one pediatric/home health nursing to patients in their homes.
• Assess and monitor patient condition, comfort, and safety.
• Develop and implement care plans in coordination with families and physicians.
• Document care activities accurately and timely in electronic health records.
