Job summary

Crossing-Healthcare is looking for an Advanced Practice Provider (APP)—an APRN or PA—to manage uncomplicated acute and stable chronic illnesses in a primary care setting. You will perform health assessments, develop diagnostic plans, and coordinate care with a multidisciplinary team.

The role requires certification as a Nurse Practitioner or Physician Assistant and current Illinois licensure. Strong patient communication and collaboration skills are essential in a fast-paced environment.

Qualifications

• Completion of NP or PA program leading to certification.
• Licensed to practice as NP or PA in Illinois.
• Experience in primary care is preferred.

Responsibilities

• Performs comprehensive health assessment through patient/family history, physical examination.
• Orders and interprets tests and diagnostic studies.
• Establishes diagnoses and treatment plans.
• Prescribes medications per collaborative agreement.
• Coordinates follow-up and referrals as needed.
• Documents all assessments, plans, and outcomes in medical records.
• Collaborates with healthcare team to manage patient care.
• Participates in quality improvement activities.
