Responsibilities

• Conduct physical therapy assessments and treat infants and young children in natural environments and via telehealth.
• Collaborate with caregivers to build practical strategies for development and daily skills.
• Maintain accurate documentation and EMR, including IFSP-related paperwork.
• Coordinate care with other therapists, educators, medical providers, and community partners.
• Participate in team meetings, case conferences, and trainings.
• Support Medicaid-related care coordination and administrative requirements.
• Contribute to staff education and professional development through trainings and sharing best practices.
