Job summary

Amazing Care Home Health in Central Texas is hiring a Registered Nurse Case Manager to lead pediatric Private Duty Nursing (PDN) services and oversee clinical care across a wide service area.

You will partner with families, clinicians, referral sources, and leadership to ensure exceptional patient outcomes while driving market growth and regulatory compliance. This role blends office and field responsibilities within a growing organization.

Qualifications

• Current Texas RN license in good standing.
• Minimum of one year of nursing experience.
• Pediatric home health or Private Duty Nursing experience preferred.
• Previous case management or leadership experience preferred.
• Bilingual Spanish preferred.

Responsibilities

• Manage a caseload of pediatric PDN patients throughout the assigned territory.
• Coordinate patient care while serving as the primary clinical resource for families and field clinicians.
• Conduct supervisory visits, patient assessments, and skilled nursing visits as needed.
• Monitor patient progress and collaborate with physicians, families, and interdisciplinary team members.
• Complete QA reviews to ensure documentation accuracy and regulatory compliance.
• Maintain complete and accurate electronic medical records.
• Conduct interviews for prospective nursing staff and support recruitment/onboarding.
• Partner with scheduling and talent acquisition to ensure clinician placement.
• Support referral development and market growth by building relationships with physicians, hospitals, case managers, and community partners.
• Represent Amazing Care at community events, networking opportunities, and career fairs.
• Collaborate with the Director of Nursing and leadership team to support operational initiatives and quality improvement efforts.
