• Strong communication and teamwork abilities.

Responsibilities

• Demonstrates knowledge of age-specific factors and works across age ranges as per unit guidelines.
• Develops and implements individualized treatment programs for patients needing services.
• Evaluates patients using data from records, interviews, observations, and team input.
• Uses activities to promote health and achieve treatment goals; adjusts plans as needed.
• Provides direct therapy services in multiple settings using varied strategies.
• Plans therapy in coordination with rehab, medical, and nursing care plans.
• Prepares documentation such as evaluations, treatment plans, and progress notes.
• Communicates patient status with family, physician, and care team.
• Establishes rapport with the interdisciplinary team and families throughout care.
• Participates in department program development and ongoing education.
