Responsibilities

• Perform evaluations and develop patient-specific treatment plans.
• Assess patient progress and adjust treatment plans as needed.
• Maintain accurate patient records per policies and procedures.
• Meet productivity targets and contribute to access to care and patient experience goals.
• Assist in program development to meet patient and community needs.
