Job summary

Spring Branch Community Health Center is seeking a Pediatric Midlevel Provider to join our team in Houston, TX. You will deliver primary care to patients from birth to 18 years, under supervision, including exams, counseling, and care coordination.

The ideal candidate holds a valid Pediatric APN license in Texas, a doctorate or master's in nursing, and at least 3 years of pediatric APN experience. Bilingual English/Spanish is a plus and EHR proficiency is required.

Qualifications

Unrestricted license to practice as a Pediatric Advanced Practice Nurse with prescriptive authority in Texas.
Doctorate or Master's in Nursing.
3 years of experience as a certified Pediatric Advanced Practice Nurse.
BLS or BLS equivalent.
Bilingual – English/Spanish is preferred but not required.
Proficiency in Computer Skills and Electronic Health Records.

Responsibilities

Provides medical services for patients from birth until 18 years of age, as delegated by the supervising Physician and Chief Medical Officer.
Prescribe medications and make recommendations for other forms of treatment.
Maintains medical expertise and competency and demonstrates knowledge of procedures to deliver quality primary medical care.
Examines, diagnoses, treats and/or refers patients for appropriate specialty care services.
Maintains a problem-oriented health record for each patient.
Completes patient charts as required by health center rules not to exceed 72 hours after the patient is seen.
Performs, orders, and interprets diagnostic studies and medical protocols.
Reviews lab and x-ray reports and makes medical management decisions.
Provides education and support to patients and addresses questions.
Collaborates with leadership and staff to assess client needs.

Summary

The Midlevel Provider for Pediatric reports directly to the Director of Pediatrics and is responsible for providing direct patient care including history review, health education, physical exam, counseling, diagnosis and treatment according to protocols and meeting productivity goals. The Midlevel Provider for Pediatric will be part of a team working in collaboration with the clinic staff to provide primary health care services effectively and efficiently.

Essential Duties And Responsibilities

Provides medical services for patients from birth until 18 years of age, as delegated by the supervising Physician and Chief Medical Officer.
Prescribe medications and make recommendations for other forms of treatment.
Maintains medical expertise and competency as well as demonstrates a thorough knowledge of procedures to deliver quality primary medical care. Provides medical sessions, as scheduled, in direct patient care at the health center.
Examines, diagnoses treats and/or refers patients for appropriate specialty care services.
Maintains a problem-oriented health record for each patient seen in accordance with the health center’s established information management policies and procedures.
Completes patient charts as required by health center rules not to exceed 72 hours after the patient is seen.
Perform, order, and interpret diagnostic studies and medical protocols.
Reviews all lab and x-ray reports for patients under his/her care on a timely basis and makes medical management decisions appropriately. Follows appropriate standards of care for each patient.
Provide education and support to patients and receive and address patient’s questions and problems.
Collaborates with Chief Medical Director, Physicians, other Mid-Level Providers, Clinical Quality Manager, Site Supervisor, Medical Assistants, Front Desk Staff, and other team members to assess the needs of clients.
Participates in the review and development of patient care policies, including health maintenance schedules and continuity of care. Serves as a resource and positive, proactive leader for the department.
Ensures that patient and family/significant others are educated about diagnosis and the plan of care. Acts as a resource for facilitating communication when necessary.
Participates in the evaluation of products, procedures, and patient care standards.Participates in conferences, workshops in specified health promotion, and education and/or prevention programs as needed; attends and participates in clinic meetings/departmental meetings and other clinic functions as required by the Health Center.
Accepts responsibility for remaining current with health care trends and information that impact the medical practice.
Performs other duties as assigned including filling in for other providers at other sites during their absences.
All Health Center staff members have emergency and disaster response responsibilities. Participates in all safety programs which may include assignment to an emergency response team.
