Job summary

Nemours Children's Health is seeking a licensed practical nurse (LPN) to contribute to patient care within a coordinated health team. You will gather data, participate in care planning, and help ensure safe administration of medications and vaccines under provider supervision.

The role requires graduation from an accredited LPN program, active Delaware licensure, and current BLS certification. You will support patients across departments and engage in continuous quality improvement efforts.

Qualifications

• Graduate of accredited Licensed Practical Nurse program.
• Active DE LPN licensure required.
• Current BLS certification required from American Heart Association.

Responsibilities

• Gather patient data and collaborate with healthcare team on the plan of care.
• Provide care that meets professional standards and regulatory requirements.
• Safely administer medications and vaccines as prescribed by the provider.
• Assist provider with procedures within the scope of the LPN license.
• Perform point-of-care testing.
• Provide age- and developmentally appropriate care to primary and specialty care patients.
• Perform and document patient intake including height, weight, head circumference, blood pressure, vision and hearing screen, reason for visit, medication reconciliation.
• Participate in quality improvement activities.
• Screen patient and family calls and escalate as needed.
