Job summary

Interim HealthCare Inc. in South Dakota is seeking an active LPN to provide nursing care in home health settings across Sioux Falls. The role includes administering medications, assisting with mobility, and monitoring patients under a plan of care.

On-call support is available and schedules are flexible to fit personal availability. Requirements include an active SD LPN license (or reciprocity), at least 18 years old, 1,000 hours of LPN experience, and pediatric experience.

Qualifications

• Active LPN license in South Dakota or eligibility through reciprocity or multi-state privileges.
• Must be at least 18 years of age.
• Previous LPN experience required - 1000 hours worked.
• 1 year pediatric experience.
• Current CPR certification required; on-site CPR certification is available for qualified candidates.
• Must maintain all required certifications, qualifications, and professional standards.
• Ability to provide compassionate, professional care in long-term care in a variety of different home environments.

Responsibilities

• Providing nursing care and supporting clients health, safety and comfort and well-being.
• Administering medications and treatments in accordance with provider orders, Interim policies and applicable regulations.
• Assisting with ambulation, transfers, and range-of-motion exercises.
• Completing nursing assessments with appropriate monitored according to plan of care.
• Monitoring and recording vital signs.
• Observing clients for changes in condition and reporting concerns to the appropriate healthcare team members.
• Identifying and reporting concerns related to patient care, medications, or medical equipment.
