Job Summary

The Pharmacist provides quality pharmaceutical care by ensuring the safe, efficacious, and cost‑effective provision of medication therapy for all patients. The pharmacist develops, recommends, and monitors evidence‑based pharmacotherapy care plans, emphasizing patient outcomes and quality of life. Care plans include assessment of patients’ medications for indication, effectiveness, safety, and adherence, as well as patient and laboratory monitoring, documentation, and outcome measurement.

This role is performed in a clinical environment, where the pharmacist is accountable for satisfying all job‑specific obligations and complying with organizational policies and procedures. Job Responsibilities

• Assumes responsibility for the delivery of pharmaceutical care in the scheduled patient care area.
• Coordinates the use of human, material, and informational resources to provide timely, efficient, cost‑effective, and appropriate care.
• Receives and interprets physicians' medication orders, clarifying or correcting orders with the physician.
• Reviews patient medication profiles to guard against drug sensitivities, contraindications, duplications, and major drug interactions.
• Reviews all medication orders for prescribing, formulary, and therapeutic issues.
• Suggests appropriate formulary alternatives based on the patient's clinical parameters.
• Consults and advises medical staff on matters related to specific drug‑therapy problems and monitors patient laboratory results.
• Provides drug information and materials to members of the medical, nursing, and other healthcare professionals.
• Provides patient discharge medication consultations based on department standards.
• Makes rounds with medical services.
• Calculates drug dosages based on pharmacokinetic parameters.
• Verifies provider‑entered orders and ensures proper doses are dispensed.
• Provides services to other healthcare providers when requested.
• Prepares special medications involving compounding procedures and sterile products using aseptic techniques.
• Reports requirements for additional supplies and equipment to maintain basic stock levels.
• Coordinates and supervises workplace activities and resources, ensuring services rendered meet departmental standards.
• Advises, assists, and supervises pharmacy technicians, students, and interns, assuming final responsibility for their work accuracy.
• Works with medical and nursing personnel to resolve pharmacy service problems.
• Coordinates activities with other pharmacy personnel to accomplish pharmacy and hospital objectives.
• Assists with the review and revision of departmental policies and procedures, as requested.

Education

Bachelor's Degree in Pharmacy (Required); Master's Degree (Preferred).

Experience

Pharmacy professional with clinical experience in a hospital or clinical setting.

Certification(s) and License(s)

Licensed Pharmacist (Pennsylvania) within 90 days of appointment; Licensed Pharmacist – Default Issuing Body.
