Responsibilities

• Provide clinical services for patients in hospital and clinics with 90% clinical effort.
• Oversee the medical care team and manage direct patient care for a defined population.
• Improve continuity of care and partnerships with community providers through timely records and communication.
• Engage in family-centered care and communicate professionally with trainees and families.
• Follow chain of command and escalation policies; respond timely to team members and external providers.
