Qualifications

• Candidates should possess excellent Patient Care skills, with a focus on family-centered, compassionate, and culturally sensitive care for children in the home setting.
• Active Registered Nurse (RN) license in New York State required; pediatric or homecare certification is highly beneficial.
• Previous experience in pediatric home health, case management, or nurse coordinator roles strongly preferred.
• Strong communication, organizational, and documentation skills, with the ability to manage multiple cases and priorities.
• Ability to work collaboratively with families and healthcare professionals and to travel to client homes as needed.

Role Description

The Pediatric Homecare Nurse Coordinator is a full-time, on-site role based in New City, NY. This role oversees and coordinates nursing care for pediatric clients in the home setting, ensuring individualized care plans are developed, implemented, and updated in collaboration with physicians, families, and multidisciplinary teams. Day-to-day responsibilities include assessing pediatric client needs, supervising field nursing staff and home health aides, and monitoring the quality and safety of care delivered. The coordinator will manage scheduling and case assignments, provide clinical guidance and education to staff and families, and ensure compliance with organizational policies, regulatory standards, and best practices in pediatric home care.
