Qualifications

• Pediatric RN or LPN license required or pending.
• Experience in pediatric home health is preferred.
• Ability to care for medically complex children at home or school.

Responsibilities

• Provide skilled nursing care in home and school settings.
• Assess and monitor patient condition and communicate changes with the care team.
• Administer medications, treatments, and prescribed therapies.
• Provide trach, ventilator, airway, and G-tube care as needed.
• Maintain accurate documentation and educate families.
• Follow individualized care plans and maintain a safe care environment.
