Qualifications

• Current RN or LPN license in MA.
• Strong knowledge of anatomy, physiology and basic life support is desirable.
• Excellent communication skills for interacting with clients and families.

Responsibilities

• Provide direct nursing care in patients' homes, including medication administration and vital signs.
• Develop and implement individualized care plans based on client needs.
• Educate clients and families about health management and disease prevention.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Document client progress and maintain accurate medical records.
• Assist with case management duties, coordinating services for clients.
