Job summary

Angels of Care Home Health is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care in patients' homes in Phoenix, AZ and surrounding areas. Both full-time and part-time positions with days, nights and weekends available.

Qualified candidates must hold an active RN or LPN license (New Grads welcome, training provided). You will care for infants, children, and adolescents, perform assessments, administer prescribed treatments, coordinate

Responsibilities

• Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the plan of care.
• Conduct ongoing patient care and assessments.
• Administration of prescribed medication, treatments, and therapies.
• Coordination of care.
• Educate family members on patient clinical care to enhance outcomes.
• Preventative initiatives to protect quality of care for patient.
