Job summary

Gentle Care Home Services seeks a dedicated pediatric home health RN or LPN to deliver one-on-one skilled nursing in the patient’s home. You will perform G-tube feedings, medication administration and site care, with ongoing clinical assessment and documentation.

The role emphasizes family education and a compassionate, family-centered approach. The schedule is Tuesday, Wednesday, Thursday from 10:00 PM to 6:00 AM, about 16 hours per week, with one patient per shift and no facility rotation.

Qualifications

• Active, unrestricted RN or LPN license required.
• Experience with G-tube feedings preferred.
• Pediatric home health experience preferred.

Responsibilities

• Provide one-on-one skilled nursing care in the patient’s home.
• Perform G-tube feedings, medication administration, and site care.
• Conduct clinical assessment and monitor for changes in condition; document accurately and timely.
• Educate and support family caregivers; communicate with the care team.
