Job summary

Angels of Care Pediatric Home Health is hiring RNs and LPNs to provide one-on-one, in-home pediatric care to medically fragile children and young adults in Sharon, PA and surrounding areas.

Qualifications

• Active, unrestricted RN or LPN license (PA).
• License acquired 1 year prior to application.
• Ability to provide care in a home setting.
• Strong communication and clinical decision-making skills.
• Pediatric or home health experience (preferred).
• Experience with high-acuity skills (trach, vent, G-tube) (preferred).

Responsibilities

• Provide skilled nursing care in a home health setting.
• Perform ongoing patient assessments and clinical evaluations.
• Administer prescribed medications, treatments, and therapies.
• Deliver care in accordance with physician orders and plan of care.
• Coordinate care with families, providers, and care teams.
• Educate patients and family members to support improved outcomes.
• Implement preventive measures to ensure patient safety and quality care.
• Respond appropriately to changes in patient condition.
