UnitedHealth Group in Hammond, LA is seeking a Per Diem Pediatric Registered Nurse to provide nursing care to pediatric patients in home settings, coordinating total care with physicians and care teams to maintain continuity.

Responsibilities include high-quality clinical services, care coordination from admission to discharge, assessing eligibility and medical necessity, and developing plans of care with community providers.

Qualifications

• Current and unrestricted RN licensure in the state of practice (Louisiana)
• Valid driver’s license and access to a dependable vehicle or public transportation
• Preferred: CPR certification or ability to complete within 90 days of hire
• 1+ years of RN experience; pediatric experience preferred
• Ability to work independently and manage time effectively, with strong written and verbal communication

Responsabilités

• Provide high-quality clinical services within scope of practice and infection control standards
• Coordinate care with the patient’s care team from admission to discharge
• Complete clinical nursing assessments per program requirements and payer needs
• Ensure patient eligibility and medical necessity for services as defined by payer and agency policy
• Develop and revise individualized plans of care with community providers
• Ensure plan of care frequency and duration meets patient needs and obtain physician approvals
• Provide care to patients in their homes regardless of age, race, creed, color, sex, disability, or financial condition
