Qualifications

• Active Missouri RN or LPN license (or Compact license) is required.

• Strong communication and assessment skills; dependable and self-motivated.

Responsibilities

• Provide compassionate, one-on-one nursing care in patients' homes.
• Administer and manage medications as ordered by the physician.
• Monitor and document patient conditions and progress.
• Perform treatments as ordered and educate families.
• Support and collaborate with the Phoenix clinical team to ensure quality care.
• Maintain open communication and scheduling with patients and families.
