Qualifications

• Must have 2+ years' experience in Maternal Child Health nursing in home care or hospital setting preferred.
• Registered Nurse license in Vermont is required.
• Registered Nurse license in New Hampshire is preferred.

Responsibilities

• Develops and coordinates care plans for patients.
• Provides ongoing assessment and communicates patient status.
• Completes clinical documentation in a timely manner.
