• Assess patients using the nursing process, including physical, social, and psychological status.
• Plan and implement an individualized care plan with patient/family involvement.
• Coordinate with the interdisciplinary team for appropriate care and education.
• Prepare patients for further support needs and follow-up as needed.
• Direct and delegate care, manage time, supplies, and resources effectively.
• Evaluate patient responses and revise care plans accordingly.
