Job summary

University of Florida College of Medicine - Jacksonville is seeking a Dietitian I to perform nutrient intake analysis and develop nutritional care plans for Ryan White patients within UF CARES and Jacksonville network. You will assess learning needs, provide age-appropriate nutrition information, and coordinate care across inpatient and outpatient settings.

The role involves discharge planning, counseling, collaboration with community resources, and documentation in the EMR.

Qualifications

• Bachelor's degree in dietetics, foods and nutrition or food service administration.
• Licenses in Dietitian in accordance with Chapter 468, Florida Statutes.

Responsibilities

• Performs nutrient intake analysis, assesses nutritional needs of patients, develops and implements nutritional care plans for Ryan White patients at UF CARES and within Ryan White Jacksonville network.
• Assesses learning needs and readiness of patients and families; provides individualized age-appropriate nutrition information to patients and families, ensuring comprehension and re-educating as needed.
• Participates in discharge planning or patient care/case conferences; follows-up on recommendations and care plans with team; coordinates and transitions care from inpatient to outpatient or outpatient to inpatient services; participates and develops pertinent nutrition related programs as needed.
• Provides counseling of patients and/or their support network when special dietary needs indicate that explanations and/or instructions are needed.
• Consults and works with community resources, professional organizations and other agencies in implementing care plans.
• Documents nutrition information in the electronic medical record. Maintains all records and reports.
• Participates as a member of the healthcare team and acts as liaison between patients, families and the clinical team to ensure quality customer service.
• Assists with developing, updating, and revising nutritional educational materials, including annual review of dietary manual.
• Develops and presents health education and promotion programs, including training workshops, conferences, and community presentations.
• Attends and participates in staffing, department and hospital-wide meetings.

Education
