Job summary

OU Health is seeking a Pediatric Care Management Social Worker for the Dialysis Clinic. The role involves comprehensive care coordination, psychosocial assessment, and family education to improve patient well-being within the OU Health system.

Responsibilities include assessments, care planning, crisis counseling, and collaboration with the healthcare team. A Master of Social Work and LMSW/LCSW licensure are required, with BLS certification and experience preferred.

Qualifications

• Master of Social Work degree from an accredited program.
• LMSW or LCSW licensure with supervision as applicable.
• Basic Life Support (BLS) certification required.
• Experience in clinical social work or healthcare settings preferred.
• Familiarity with CMS regulations and care coordination.

Responsibilities

• Assess patients’ conditions using medical data, interviews, and care team input.
• Perform psychosocial assessments and relate findings to clinical staff.
• Develop and monitor care plans including referrals and crisis interventions.
• Educate patients and families about diseases, treatments, and resources.
• Monitor progress toward goals and adjust interventions as needed.
• Provide crisis counseling and document mandated reports.
• Act as liaison between patients, families, and healthcare providers.
• Advocate for patient concerns and ethical considerations.
• Mentor LMSW/LCSW trainees and supervisees when applicable.
