Job summary

Valley Children’s Healthcare is seeking a Diabetic Educator-RN to support pediatric patients with diabetes and their families. You will assess learning needs and deliver developmentally appropriate education to promote self-management.

You will collaborate with physicians, nurses, educators, and other professionals to implement family-centered care, document outcomes, and serve as a clinical resource for diabetes education while adhering to California Nursing Practice Act.

Qualifications

• Registered Nurse (RN) licensed by the California Board of Registered Nursing.
• Associate Degree in Nursing required; BSN preferred.
• BLS certification through the American Heart Association.
• CDCES certification preferred.
• Minimum 1 year of related nursing experience; 2+ years preferred.

Responsibilities

• Assessed patients’ and families’ educational needs and developed individualized, age-appropriate, culturally responsive education plans.
• Provided diabetes education and ongoing support to patients and families to promote effective disease management and healthy outcomes.
• Collaborated with physicians, nurses, educators, and other healthcare professionals to develop and implement family-centered, outcome-based plans of care.
• Served as a clinical resource and consultant for diabetes-related care and education.
• Documented patient assessments, education, interventions, and outcomes in accordance with organizational standards.
• Supported patients and families in developing the knowledge and skills needed to safely manage diabetes.
