Affirmed Home Care, Inc. in Rochelle Park, NJ, is seeking an experienced Clinical Executive Director (RN) to lead pediatric clinical operations across New Jersey, New York, and Connecticut.

This senior role emphasizes high-acuity pediatric home care, regulatory compliance, and building strong nursing teams, with travel between offices as needed. The ideal candidate will provide hands-on leadership during start-of-care visits, approvals, and QA/QAPI initiatives, while partnering with Billing,

Qualifications

• Active Registered Nurse NJ (RN) license.
• 5+ years of pediatric home care leadership experience preferred.
• Strong knowledge of pediatric home care, clinical documentation, and regulatory requirements.
• Experience with Medicaid, Medicare, managed care, authorizations, and payer requirements.
• Strong leadership, communication, organization, and problem-solving skills.
• Ability to travel between NJ, NY, and CT
• Flexibility to support occasional evenings and weekends as needed.

Responsibilities

• Lead and oversee pediatric clinical operations across NJ, NY, and CT.
• Supervise pediatric RNs and LPNs with clinical guidance.
• Manage caseload of pediatric clients, including high-acuity cases.
• Lead start-of-care visits, assessments, supervisory visits, discharges.
• Ensure plans of care, physician orders, assessments, visit notes compliance.
• Conduct chart reviews to ensure DOH/Medicare/Medicaid compliance.
• Oversee authorizations, SCAs, LMNs to support continuity of care.
• Monitor authorizations and address renewals and denials.
• Participate in QA/QAPI initiatives to improve quality and reimbursement.
• Support nursing orientation, coaching, and performance management.
• Collaborate with Billing, Compliance, HR, physicians, hospitals, insurers.
• Manage on-call coverage and staffing during off-hours.
• Support recruitment and development of pediatric nursing teams.
