Qualifications

• Current license as an Advanced Practice Registered Nurse (APRN) in CT.

• Experience delivering advanced nursing care to diverse populations.
• Experience in chronic disease management as an APRN.

Responsibilities

• Provide comprehensive advanced nursing treatments, diagnostics, and treatments.
• Lead and participate in multi-disciplinary teams and care plans.
• Consult with clinical staff on assessment methods and treatments.
• Prescribe medications and monitor responses; adjust as needed.
• Provide individual psychotherapy and health education to patients.
