Job summary

Yale NewHaven Health in New Haven, CT is seeking an experienced Nurse Coordinator to join our pediatric specialty teams. As a Registered Nurse, you will coordinate patient care across the continuum, guided by nursing leadership and Attending Physicians.

You will educate patients and families, collaborate with a multidisciplinary team, participate in quality improvement and data collection, and uphold the YNHHS values of integrity, patient-centered care, respect, accountability and compassion.

Qualifications

• Bachelor's degree in Nursing is required.
• Two to three years of RN experience preferred in pediatrics or nurse coordinator roles.
• Connecticut RN license is required.

Responsibilities

• Lead member coordinating all aspects of patient care for the specific pediatric specialty.
• Provide patient and family education on condition, treatment regimens, signs and potential adverse outcomes.
• Collaborate with the multi-disciplinary team to coordinate care across the continuum.
• Participate in data collection related to clinical and operational efficiencies.
