Summary

Responsible for following an individual patient care plan to assist patients and/or perform personal care when an individual or family cannot perform the tasks. The aide will care for patients with all types of diagnoses and in many different phases of illness.

Responsibilities

A Day in the Life of a LTC Nurse Aide / PCT at MyMichigan Health:

• Provides hands‑on personal care and demonstrates competency with assigned tasks; follows the patient’s plan of care for completion of tasks. Performs simple procedures as an extension of nursing services. Assists in ambulation or exercises. Complies with infection prevention and control policies and procedures. Creates a positive relationship with the patient and family. Honors patient rights. Is a member of the interdisciplinary team, reports changes in the patient’s condition to a registered nurse, and completes pertinent records in compliance with policy and procedures.

Other Duties and Responsibilities
  • Promotes patient independence and assists or performs personal care (bath, dressing, shampooing, shaving, oral care, skin care, toileting, changing bed linen and other personal care activities). Accurately measures and documents temperature, pulse, respirations, and blood pressure when required per care plan, and promptly reports abnormal findings to appropriate personnel.
  • Promotes skin integrity by keeping skin clean and dry, using pressure‑preventative positioning per care plan. Observes any alteration in skin integrity and reports changes promptly to nurse or manager.
  • Utilizes proper positioning techniques when repositioning the patient in bed, with ambulation or while transferring in/out of bed or chair, maintaining a safe and secure environment throughout care provision.
  • Follows the patient’s plan of care and accurately documents care provided and any other pertinent information concerning the patient and/or family. Documentation is completed timely per policy and state requirements.
  • Consistently demonstrates a positive, courteous, and helpful attitude toward patients, families, and all customers during all encounters. Functions as a team member through positive verbal and non‑verbal communication to support positive relationships and achieve organizational goals.
  • Maintains knowledge of and follows organizational policies & procedures. Maintains knowledge and skills regarding the use of equipment.
  • MyMichigan Health is technology‑driven and employees must demonstrate competency in Microsoft Windows. Performs other duties as assigned.
Certifications and Licensures

BLS – Basic Life Support

Preferred Equivalent Experience – required within 1 month of hire or transfer

CENA – Certified Nurse Aide – required within 4 months of hire or transfer

FingerPrinting – Fingerprinting Required

Requirements We Are Looking For

Education: High School Diploma or GED

Experience, Training and Skills
  • Attend meetings/in‑services as required; complete no less than 12 hours of continued education/in‑service training as defined by the organization during a 12‑month period. May be required to participate in further learning opportunities offered by MyMichigan Health. Ability to communicate effectively, verbally and in writing, and to speak, write, and understand English.
  • Ability to relate to individuals of all ages and with different cultural and spiritual backgrounds.
  • Ability to work with patients and families dealing with terminal illness, the dying process, and death.
Physical/Mental Requirements and Typical Working Conditions
  • Exposure to stressful situations, including public contact, trauma, grief, and death.
  • Able to wear personal protective equipment that includes latex materials or appropriate substitutes if required for the position.
  • Can move freely around the facility with or without an assisted device and can perform the functions of the job as outlined in the description.
  • Vision and hearing are necessary, with or without assisted devices.
  • Required to sit/stand/walk for long periods; may need frequent postural changes such as stooping, kneeling, or crouching.
  • May be exposed to blood‑borne pathogens and other potentially infectious materials; must follow MyMichigan Health bloodborne pathogen and TB testing requirements.
  • Ability to handle multiple tasks, work independently, maintain regular attendance, and stay alert.
  • Requires overall dexterity, including handling, reaching, grasping, fingering, and feeling; may require repetitive movements regularly.
  • Physical demand level: Medium. Occasionally (0‑33% of the workday) lift or carry 21‑50 lbs.; frequently (34‑66% of the workday) lift or carry 20 lbs.; regularly (67‑100% of the workday) lift or carry 10 lbs.
