Comagine Health in Oregon seeks a qualified Home and Community Based Coordinator to support individuals with serious mental illness. You will discharge planning, coordinate services, and collaborate with providers and community agencies to ensure supports are in place for the least-restrictive settings.
The role requires QMHA certification, a Bachelor’s degree, 2+ years of clinical experience, and 1+ year of care coordination experience preferred.
FOCUS Areas: Eugene and Portland Metro, but will review qualified candidates from all areas in Oregon.
Comagine Health partners with the Oregon Health Authority (OHA) to provide statewide eligibility determination, service coordination, medical appropriateness reviews, and transition planning for individuals diagnosed with serious and persistent mental illness (SPMI). Our goal is to support adults with SPMI to live in the least-restrictive settings possible by ensuring that necessary and desired supports and services are in place. The Home and Community Based Coordinator facilitates timely and appropriate discharges and care transition support services through collaboration with providers, community service agencies, and other stakeholders.
Qualified Mental Health Associate (QMHA) certification is required.
