Job summary

MercyOne Central Iowa is seeking a Patient Logistics RN to coordinate complex patient transfers across the healthcare continuum in the Des Moines metro area. You will work with referring hospitals, accepting facilities, and transport teams to ensure timely, safe patient movement and accurate clinical information.

Strong clinical judgment, exceptional communication, and proficiency with electronic health records are essential to improve patient outcomes and facility throughput in this off-site

Qualifications

• Current RN license or equivalent licensure.
• Minimum 2-3 years of clinical nursing experience.
• Critical care, emergency, or medical-surgical experience preferred.
• Strong communication and interpersonal skills.
• Experience with electronic health records and transfer systems.
• Knowledge of regulatory requirements for patient transfers.

Responsibilities

• Coordinate and facilitate patient transfers between facilities, departments or units.
• Operate in a fast-paced, ever-changing environment.
• Assess transfer requests to determine level of care required.
• Communicate with physicians, nurses, and staff at sending and receiving facilities.
• Manage multi-phone activity to minimize delays.
• Document transfer information in electronic health records.
• Coordinate transportation including ground and air services as needed.
• Monitor bed availability and collaborate with bed management.
• Serve as a clinical resource for transfer questions.
• Participate in quality improvement initiatives related to transfers.
• Activate emergency notification system for STEMI, stroke, trauma, ECMO when needed.
