• Own the patient activation process from lead to onboarding.

• Coach and develop Patient Success and Care Concierge teams.
• Refine processes to maximize patient conversion and retention.
• Collaborate with cross-functional teams for seamless coordination.
• Track and report key performance metrics for team improvements.
