The ideal candidate will have a high school diploma and either an LPN/LVN license with clinical experience or recent home health/hospice/pediatrics exposure, plus basic tech proficiency including mobile devices.

Qualifications

  • Must possess a high school diploma or equivalent.
  • Must either be a licensed practical or vocational nurse with at least one year of clinical experience, or have at least one year of home health, hospice, or pediatrics within the last 24 months.
  • Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

  • Schedule patients to branch field clinicians and coordinate care.
  • Communicate with field staff, patients, physicians, referral sources, caregivers, and service providers to maintain care coordination.
  • Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
