Job summary

McLaren Health Care in Michigan is seeking a Nursing Assistant for per diem shifts to provide close, continuous observation of at-risk patients under supervision of an RN, ensuring safety and a clean environment.

Responsibilities include receiving verbal reports, staying at bedside, assisting with activities of daily living, and communicating professionally with the care team; weekends required.

Qualifications

• High school diploma or GED.
• Completion of a nursing assistant program.
• One year related experience.

Responsibilities

• Obtain verbal report from nurse including care needs, diagnosis, barriers, and special considerations.
• Stand at bedside to maintain constant visual observation of the patient.
• Follow nurse instructions for ambulation and redirection as needed.
• Accompany patient off unit for tests, staying within arm's length unless directed otherwise.
• Maintain a safe, clean, comfortable environment for patients.
• Work as part of a team and adjust to patient/unit needs; assist as needed in emergencies.
• Communicate professionally with patients, families, and health-care staff.
• Record data accurately and notify RN of untoward events or changes in behavior.
• Perform other duties as assigned by nursing staff.

Position Summary:

Under the supervision of an RN, provides close, continuous observation of patients who may be at risk for adverse events such as falls, self-injury or harm to others.Assists in the provision of a safe and clean environment. Provides assistance with activities of daily living, unless the patient needs are such that adverse health consequences are predictable.

Essential Functions and Responsibilities as Assigned:

• Obtains verbal report from the patient’s nurse to include information regarding the patient’s identified care needs, diagnosis, visual, auditory, or language barriers, as well as any special consideration (NPO, bed alarms, etc.).
• Positions self in patient room at bedside to maintain constant visual observation of the patient. Never leaves the patient alone or out of sight unless specifically instructed by the nurse.
• Obtains instruction from nurse regarding ambulation limitations and course of action for redirection of patient if needed.
• May not transport patient for testing and procedures off the unit but must accompany and remain within arm’s length of patient unless otherwise directed by the person performing the test or procedure.
• Provides for patient comfort, safety, and satisfaction.Maintains patient environment in a safe and orderly manner.
• Functions cooperatively as a team member.Responds as directed by responsible nurse to changing patients'/unit's needs.During a patient or environmental emergency, may serve as a runner or transport patients as directed.
• Communicates in a professional manner and demonstrates respect for patient, families/significant other and other health care members.
• Accurately and legibly records data collected and notifies RN of any untoward events, i.e., behavioral changes, violent behavior, etc.
• Performs other related duties as required and directed.
