Job Summary and Qualifications

As a Nurse Navigator , you will function as a member of the multidisciplinary team and serve as an advocate and educator for patients with all types of medical conditions. You will be responsible for support ing patients through complex healthcare environments for the optimization of care and outcomes . You will facilitate patient follow-up through coaching, information transfer, and communication to improve continuity of care, enhance the medical management of complex diseases, and streamline the intervention process . You will also guide patients and families through the healthcare system by assisting with access issues to expedite the timeliness of care and treatment. You will achieve this goal by building relationships with patients and physicians, coordinating the defined plan of care, assisting with appointments, assisting in resolving barriers, providing education, and connecting with community resources. You will also assume responsibility and accountability for the management of resources to achieve efficient, high-quality outcomes for patients, including support with interdisciplinary and cross-facility coverage and collaboration. You are responsible for timely data collection that promotes tracking of the navigation program performance in line with HCA Healthcare, division , and facility goals (growth, quality, practice guidelines, etc ).
